This Privacy Policy explains how we handle the personal information that you provide to us on web sites controlled by Allos Therapeutics, Inc. (“Allos”) which link to this Privacy Policy (referred to as “Allos web sites”).  This Privacy Policy does not apply to personal information collected from you offline, to Allos web sites that do not link to this Privacy Policy, or to third-party web sites to which Allos web sites may link.  Your use of Allos web sites which link to this Privacy Policy is subject to this Privacy Policy and the Terms of Use.  If you reside outside of the United States, please be aware that the Allos web sites are intended for use by United States residents only, and information will be used in accordance with United States law, which may provide less protection than the laws of other countries.

How does this Privacy Policy define “personal information”?

“Personal information” means information that identifies you or could reasonably be used to identify you and which is submitted to and/or collected by Allos web sites and maintained by Allos in an accessible form.  Examples of personal information include your name, your postal address and your telephone number.  Data that are not personal, such as questions, comments, ideas and suggestions excluding personal information, shall be deemed to be non-confidential, and Allos shall be free to disclose and use the same, without any obligation whatsoever to you, for any purpose and through any means.  This Privacy Policy describes the treatment of personal information.

How does Allos collect information online?

Allos collects information in two ways:

Personal Information You Submit:
Allos collects personal information that you enter into data fields on Allos web sites.  For example, you may submit your name, postal address, e-mail address, and/or other information in order to receive information about various subjects, register for Allos programs, contact Allos customer service, or respond to Allos surveys.  To protect your privacy, you should not provide Allos with any information that is not specifically requested, such as your medical history.

Passive Collection of Unidentifiable Information:
Allos web sites may collect information about your visits to Allos web sites without you actively submitting such information.  Unidentifiable information may be collected using various technologies, such as cookies, Internet tags, and web beacons.  Your Internet browser automatically transmits to Allos web sites some of this unidentifiable information, such as the URL of the web site you just visited and the browser version your computer is operating.  Passive information collection technologies can make your use of Allos web sites easier by allowing Allos to provide better service, customize sites based on consumer preference, compile statistics, analyze trends, and otherwise administer and improve Allos web sites.  Certain features of Allos web sites may not work without use of passive information collection technologies.  Information collected by these technologies cannot be used to identify you without additional identifiable information.

In what circumstances might Allos collect and use personal information?

Allos will use the personal information you provide through Allos web sites to respond to your questions, to provide you with efficient customer service, as otherwise described at the time you provide your information or in subsequent notices to you, or for other Allos business purposes.   After you have entered personal information into a form or data field on an Allos web site, Allos may use certain identifying technologies, referred to as “cookies,” to allow that web site to “remember” your personal preferences, such as sections of that web site that you visit frequently and, if you choose, your user ID.  We may also use personal information you provide or that we obtain about you for other business purposes, such as to offer you the opportunity to receive notices regarding Allos products or services, to invite you to participate in surveys about our products, or to notify you about special promotions.

Does Allos consolidate personal information?

Allos may consolidate the personal information of customers who use Allos web sites.  We use this information to help us better design Allos web sites and Allos products, to communicate information to you, to enhance our marketing and research activities, and to facilitate other business functions.

What choices do I have about how Allos collects and uses personal information about me?

You can access and use much of the Allos web sites without telling us who you are or revealing any personal information.  Some areas of the site, however, do allow you to contact us to ask questions or provide comments.  You may always limit the amount and type of personal information that Allos receives about you by choosing not to enter any personal information into forms or data fields on Allos web sites; however, some of our online services can only be provided to you if you provide us with appropriate personal information.  You may also, at the time you receive a marketing email message from Allos, follow the instructions in the email message to opt out of receiving future such marketing email messages.
You may also be provided with preference questions or preference boxes allowing you to indicate that you do not want Allos web sites to use tracking technologies, such as cookies, to “remember” your personal information, such as user IDs or mailing addresses, on return visits.  However, Allos web sites that use tracking technologies to collect unidentifiable information do not generally provide you with the ability to opt out of the tracking technologies.  Some Internet browsers allow you to limit or disable the use of tracking technologies that collect unidentifiable information, erase cookies from your computer's hard drive, block all cookies, or even receive a warning before a cookie is placed on your system.  Refer to your browser instructions or help button to learn more about tracking technologies and cookies.

Does Allos share personal information with (non-governmental) third parties?

Allos shares personal information with third parties business partners, such as contractors who are providing services to Allos, or to the extent applicable, third parties with whom Allos is conducting a joint program.  If Allos sells one of its product lines or divisions, your personal information may be transferred to the buyer so that the buyer can continue to provide you with information and services.  Allos will also disclose your information to its affiliates and parent companies, to protect its interests or the company, or if it believes it is otherwise required to do so by law, or if doing so will not violate any law.

Does Allos share personal information with Regulatory or Law Enforcement Authorities?

Allos will disclose your personal information if it believes it is required to do so in response requests from government authorities, to address national security situations, or when otherwise required by law.  If you contact Allos regarding your experience in using one of Allos’ products, Allos may use the information you provide in submitting reports to the Food and Drug Administration (FDA), other regulatory authorities, and as otherwise required of us by law.  Allos also may use the information to contact the prescribing physician to follow up regarding an unexpected event involving use of our product.  By submitting such information, you agree that Allos may not be permitted to comply with your request to amend or remove personal identifiable information that was provided to Allos by a health care professional or a consumer regarding an adverse drug event.  Additionally, Allos may be required by court orders or other laws to divulge information about you to law enforcement authorities or the courts.  Allos will cooperate in responding to such requests, in accordance with the law, and will take appropriate measures to ensure that the requester understands the sensitive nature of the health-related personal information that they may receive.

What security does Allos use?

It is Allos’ practice to use reasonable security measures on its websites, including web pages that collects personal information; however, the confidentiality of personal information transmitted over the Internet cannot be guaranteed.  We urge you to exercise caution when transmitting personal information over the Internet, especially personal information related to your health.  Allos cannot guarantee that unauthorized third parties will not gain access to your personal information; therefore, when submitting personal information to Allos web sites, you must weigh both the benefits and the risks.  In addition, Allos web sites covered by this Privacy Policy will display a warning whenever you link to a web site that is not controlled by Allos; you should check the privacy policies of such third-party web sites before submitting personal information.

Are the Allos web sites intended for children?

No, the Allos web sites are not intended for, nor does Allos believe that they are appealing to, children.  Allos does not knowingly collect or use any personal information from children (we define “children” as minors younger than 18) on Allos web sites.  We do not knowingly allow children to order our products, communicate with us, or use any of our online services.  If you are a parent and become aware that your child has provided us with information, please contact us using one of the methods specified below, and we will work with you to address this issue.

In all communications to Allos, please include the e-mail address used for registration (if applicable), the web site address on which you provided any personal information about which you have questions or concerns (e.g., www.allosbio.com) and a detailed explanation of your request.  If you would like to delete or amend your personal information and are contacting us by e-mail, please put “Deletion Request” or “Amendment Request”, as applicable, in the subject line of the e-mail.  We will do our best to respond to all reasonable requests in a timely manner.

How will I know whether Allos has updated this policy?

Allos reserves the right to amend this Privacy Policy without prior notice to reflect technological advancements, legal and regulatory changes and good business practices.  If Allos changes its privacy practices, a new Privacy Policy will reflect those changes and the effective date of the revised Privacy Policy will be set forth in this paragraph.  This Privacy Policy is effective as of May 27, 2009.


Allos Therapeutics, Inc. does not guarantee coverage and/or reimbursement for FOLOTYN. Coverage, coding, and reimbursement policies vary significantly by payer, patient, and setting of care. Actual coverage and reimbursement decisions are made by individual payers following the receipt of claims. Allos strongly recommends verifying coverage, coding, and reimbursement guidelines on a payer and patient-specific basis.


IMPORTANT SAFETY INFORMATION

Warnings and Precautions

  • FOLOTYN may suppress bone marrow function, manifested by thrombocytopenia, neutropenia, and anemia. Monitor blood counts and omit or modify dose for hematologic toxicities.
  • Mucositis may occur. If ≥Grade 2 mucositis is observed, omit or modify dose. Patients should be instructed to take folic acid and receive vitamin B12 to potentially reduce treatment-related hematological toxicity and mucositis.
  • Fatal dermatologic reactions may occur. Dermatologic reactions may be progressive and increase in severity with further treatment. Patients with dermatologic reactions should be monitored closely, and if severe, FOLOTYN should be withheld or discontinued.
  • Tumor lysis syndrome may occur. Monitor patients and treat if needed
  • FOLOTYN can cause fetal harm. Women should avoid becoming pregnant while being treated with FOLOTYN and pregnant women should be informed of the potential harm to the fetus.
  • Use caution and monitor patients when administering FOLOTYN to patients with moderate to severe renal function impairment.
  • Elevated liver function test abnormalities may occur and require monitoring. If liver function test abnormalities are ≥Grade 3, omit or modify dose.

Adverse Reactions

  • The most common adverse reactions were mucositis (70%), thrombocytopenia (41%), nausea (40%), and fatigue (36%). The most common serious adverse events are pyrexia, mucositis, sepsis, febrile neutropenia, dehydration, dyspnea, and thrombocytopenia.

Use in Specific Patient Population

  • Nursing mothers should be advised to discontinue nursing or the drug, taking into consideration the importance of the drug to the mother.

Drug Interactions

  • Co-administration of drugs subject to renal clearance (e.g., probenecid, NSAIDs, 
and trimethoprim/sulfamethoxazole) may result in delayed renal clearance.
